We target the most challenging patients — many with more than one chronic illness. Their claims typically exceed $30k/year. Tell us who they are and we will knock on doors and find them, and offer them a welcoming place to get the quality, coordinated care they so desperately need.
Our Ambulatory ICUs make it easy for members to get the care they need. Walk-ins are welcome. They’re close to public transportation. Parking is free. And if they can’t get to us on their own, we provide door-to-door transportation.
Members are assigned their own multidisciplinary team, including a primary care physician, a care manager and social and behavioral experts to help them navigate any barriers in their lives. We do not work in silos. We work together, fully focused on the entirety of each member’s needs. The average PCP sees 25 or more patients a day. Ours see approximately 10.
Physicians, counselors, pharmacists, x-ray techs and other providers are not across town from each other. They’re down the hall.
Many members endure extreme social and psychological hardships that exacerbate chronic health conditions and complicate care access and delivery, so we provide counseling and outreach services designed to help our members live healthier, happier lives.
