AbsoluteCARE, a patient-centered ambulatory ICU providing care to the most complex chronically ill patients aged 16 and older, has opened its third innovative ambulatory ICU medical office in Greenbelt, Maryland. This facility welcomed Prince George’s County’s most hard-to-treat population in September.
AbsoluteCARE uses a well-coordinated, specialized “health home” to best provide for the complex care needs of chronically ill patients. In addition to providing outstanding medical care, this unique, multi-dimensional model features case managers, social workers, an integrated behavioral health team, educators, an on-site laboratory, an in-house pharmacy, and infusion services. The first location opened in Atlanta in 2000. Downtown Baltimore’s center at 1040 Park Avenue opened in 2014. All three locations boast a warm, friendly atmosphere and the latest in medical technology.
“Just as we do in Baltimore and Atlanta, AbsoluteCARE will significantly improve access, quality, outcomes, and lives for the most vulnerable populations here in Greenbelt,” said Dr. Greg Foti, Executive Vice President, Innovative Medical Operations. Dr. Foti is responsible for the health management initiatives and is deeply invested in innovative medical models and how best to leverage technology to improve the delivery of health care, improve access, and increase quality. Dr. Foti added, “We provide value and significant savings for our partnering health plans while truly caring for our patients.”
Compassion, confidentiality, comprehensive care, and cultural competency are core values of AbsoluteCARE. Their Ambulatory ICU approach helps to drive down unnecessary ER/hospital admissions by more than 40 percent while increasing quality and value-based purchasing and HEDIS measures from the low 30th percentile to the 80 to 90th percentile. Most important, they change the lives of their patients and reverse the systemic sickness that plagues the next generations of these underserved populations.
AbsoluteCARE will open a fourth location in Philadelphia in the first half of 2018.
